The completion of this form is required for returning product to Skurka Aerospace, Inc. for any reason. After completion, transmit this request to Skurka Aerospace by pressing the "submit" button at the bottom of the form. A RMA number will be assigned and returned by our Quality Assurance Department. After the assignment of the RMA# please use this number for all communication. Order status of customer returns must be directed to the Sales Department at (805) 484-8884 or sales@skurka-aero.com.

Please clearly label the container and all associated paperwork with your assigned number. Failure to label the container may result in non-acceptance at our Receiving dock.

Applicable Terms:

1). This RMA number expires within 30 days from the date of issue. 2). All products must be returned to Skurka Aerospace Inc. prior to the expiration date, transportation pre-paid, in strict compliance to our warranty policy for our evaluation and determination of responsibility. 3). The return shipment must include the product, a copy of the original Skurka Aerospace Inc. shipper and the RMA number clearly labeled on the carton to prevent refusal at our receiving dock. 4). Should non-conforming material be deemed not to be Skurka Aerospace Inc.’s responsibility you will have fourteen (14) days to respond to our notification for disposition, or material will be returned to you “as-is” (evaluation/test charges may apply). 5). All responses for corrective action will be on Skurka Aerospace Inc. forms and format. A quotation can be provided to transcribe the Skurka corrective action report on customer’s forms. Corrective action requests for products out-of-warranty will be a chargeable unless waived by our Sales Department.  6). Additional Charges may apply for failure analysis reports.
